- Age
- Birth through 20
- Needs
- Has or is at risk for physical, developmental, behavioral or emotional conditions
- Income
- Title V funding: SSI or TEFRA recipients under 18, first-come first-served, based on need and financial eligibility

- Help with applications
- Title V funds for needs not otherwise covered (reported up to $2,000)
